Your First Visit

❖ Please plan to arrive 30 minutes early for your appointment. This allows time to review the paperwork. If you prefer, you can download the forms in advance.


❖ It is helpful to bring along a list of all the medications you are taking, including prescription, over-the-counter, and herbal medications, along with supplements. It is important that we consider your symptoms in the context of your entire medical condition.


❖ If you have previously had mental health treatment, it is also helpful to bring a list of your past treatment experiences.


❖ Your appointment will last approximately 50 minutes. You will have a chance to discuss your problems and symptoms in depth. You’ll be asked about your medical history, family history, and social situation. At the end of the visit, we will discuss a possible diagnosis and agree on treatment together. Treatment recommendations may include medications, therapy (with Dr. Mui or with another provider), or other interventions. If needed, you may also be asked to have laboratory tests to check on medical conditions or to monitor medications.

Future Visits

❖ You will be scheduled for a 25-minute or a 50-minute appointment, depending on what we have decided together.


❖ It is helpful to bring along a list of any medication changes or any changes to your general medical condition.


❖ You will be asked about how you are doing, how the treatment is working, and whether you are having any problems with it. We will discuss any changes in your situation and how those may be affecting you. We will set treatment goals together and monitor your progress. Even if you are not receiving formal therapy in the clinic, we will talk about coping skills and strategies.


❖ You will be given an opportunity to ask questions and discuss any other specific concerns you have.

Cancellations

* If you need to cancel your appointment, please notify the clinic by 3:00 PM of the business day before your appointment. This allows us to accommodate other patients who may be waiting. You will be charged a fee for late cancellations or missed appointments.

Payment

❖ Please plan to pay at the time of your appointment. Payment can be made with checks or cash.


❖ The clinic does not accept insurance, but we can provide you with a claim form that you can submit to your insurance company if you choose. If you are planning to submit a claim, you may want to review your policy's coverage for an out-of-network psychiatrist, so that you can anticipate your responsibility. For patients with Medicare, it is important to understand that Medicare will not reimburse you for your costs.
